Judd Coffman, a Patient Care Pharmacist at Jennie Edmundson, Honored With the Good Catch Award

The Good Catch Award is designed to recognize staff members committed to patient safety.

This recognition is presented regularly and goes to the department in which the good catch occurred. Nominations for the award are taken from reports that indicate a near miss was caught. Those catches are then flagged for consideration for the award. A near miss is an incident or event that has occurred and had the potential to cause harm but was caught before it reached the patient.

A patient was admitted to the ICU in critical condition. During the medication reconciliation, there were concerns that the patient had been taking a medication that was been previously listed. Judd worked diligently and was able to identify that the patient had been receiving another patient’s medication after it had been dispensed from an outside pharmacy. This wrong medication led to the patient’s critical condition. Since the identification of the error, the patient has made a full recovery.
