The following information outlines what to expect and bring with you to the appointment. Please note that missing items at your appointment may delay your start date.
If you are under the age of 18, you must have a parent or guardian available in person or via phone at the time of the appointment for consent of drug screening test. You must also have the second page of the DHHS Registry form printed. Both your signature and your parent or guardian's signature must be notarized by a notary public. We have a notary public available on site if your parent or guardian comes to your appointment.

What to Bring to Appointment
- Immunization Records
- Identification Documents for I-9 Employment Eligibility Form
- List of Acceptable Documents You may present one selection from List A or a combination of one selection from List B and one selection from List C.
- Documentation for bank account, which is required for employment (voided check, direct deposit form or letter from bank).
- Don't have a bank account? Check out the flyer below to obtain a bank account or join a credit union.
- Certifications and Education documentation for verification (if instructed by the HR Representative)
- Please complete all employment forms prior to your appointment
- New Employee Orientation (NEO) - Information Orientation information is provided following completion of the pre-employment process and is to be completed within thirty days from your employment start date.

You have 30 days from your hire date to enroll in benefits. The benefits you elect will be effective on the first day of the next month from your start date.

Section 1: Employees Scheduled 20 or More Hours per Week

Section 2: Employees Scheduled 16 to 19 Hours Per Week

Section 3: Information for Employees with No Scheduled Hours
As a casual employee with no regularly scheduled hours, you are eligible for the following benefit:

Methodist Health System requires vaccination for all employees but offers exemption to those who meet the CMS, EEOC and ADA guidelines for exemption. The medical and religious exemption request forms are linked below.
